Friedman, Cecylia born 1/7/1913 [July 1] in Krakow who is single of profession trade assistant in Krakow resides at Dietla 34 responsible for ____ is personally known [to the witnesses].
We confirm the identity of the person in the photograph below.
We explain the above for the purpose of obtaining an identity card for the Jewish Communist of Krakow in regard to relaocation from Krakow to ______.

The expression "zuständig nach" tranדlated above as "responsible for" is also used to identify the place where a person belongs to bureaucratically - usually where their birth was registered.
I suggest that this is its meaning in the present form.
