A bit tricky without the headings, but this is what I reckon.

In column order:

Date and place of birth: 1 April 1901, Krakow, 28 Sebastyana Street.

Date and place of naming ceremony: 6 April, 1901, Krakow. There's a notation underneath saying RICHTER ??, but I don't know what this refers to.

Name of child: Berta

Gender: daughter

Legitimate / Illegitimate: Illegitimate

Father: Mojzesz-Isser NEULINGER, with his parents in Krakow.

Mother: Ruchel FRIEDMANN of Krakow, daughter of Mojzesz FRIEDMANN, handlarz zelarza (iron merchant) of Krakow and his wife Doba.

Officiant: Chaim Jakob SLESSNER? of Krakow as cantor.

Midwife: Rozalia VOGLER of Krakow.

Comments: I declare that I am the father of this child, Krakow, dated April 15, 1901.
