Not a very clear image, but I've tried my best. Just fyi, no professions mentioned for the parents.

In column order:

1. Record #: 42

2. The birth: 30 June 1909, in Chorostków (Khorostkiv), house #74.

3. The circumcision / naming ceremony: 7 July 1909, Chorostków, house #74.

4. The child: Osias, male

5. Status: illegitimate

6. Father: Juda Chaim TUNIS, currently living in Chorostków, marriage according to the laws of Moses to Feiga MESSING, inhabitant of Hylcze?

7. Mother: Feige MESSING, marriage according to the laws of Moses, inhabitant of Hylcze?, daughter of Leizor and Cipie? MESSING, stallholders in Chorostków.

8. Godfather/sandek/witness: Naftaly GLÜCK from Chorostków, sandek [the person who holds the baby during the bris]

9. Mohel: Fischel KOHN

10. Midwife: Gitel EUGLER

11. Stillbirth: -

12. Comments: Stamp of the district office of Husiatyn, 8 July 1909

The note across columns 4 and 5 says something along these lines: Pursuant to a rescript from the Voivodeship Office in Tarnopol dated May 21, 1931 (Ref. No. AD. 2244), and a subsequent directive from the District Office (Starostwo) dated June 3, 1931 (Ref. No. 2586/31), the following correction is made: The paternal surname of this child is Horwitz, not Tunis.
