Full translation requested, but four specific fields matter most:
The father's given name (line 3, before "Мстовский/Метовский"). I cannot resolve it. It is not Ejzyk — it has a long medial descender. A shape consistent with Юдка/Judka, but that is a guess.
The surname — Мстовский or Метовский? JRI-Poland indexes it "METOWSKY / MSTOWSKI?" with a question mark, and the book's own index reads Метовский. But the TOWN in lines 1 and 5 seems written the same way, so this may just be the clerk's spelling of "of Mstów."
The mother's maiden name (line ~8, after "урожденной"). I read ГАБЕЛЬМАН / Gabelman — confirmation or correction welcome.
The father's occupation — I read подрядчик (contractor). And his stated age.
