Addendum for Donating Previously Published Material to a JewishGen Project

Due to possible restrictions on previously published material, the following information is required for all such material, including translations of all books and databases.  Please download, complete, and attach to your donor agreement.  Any questions regarding completion of this page, please contact the Project Coordinator for the project to which you are donating.
